For the past year, the National Academy for State Health Policy (NASHP) has worked with a group of eight states (Alabama, Iowa, Kansas, Maryland, Montana, Nebraska, Texas, and Virginia) to develop medical home programs. Over the past 12 months, these eight states, referred to as the Medical Homes II Consortium, have made strides toward this goal. This webcast, featuring policymakers from the eight states, will highlight a variety of their achievements in the areas of forming partnerships, defining and recognizing medical homes, reforming payment, supporting practices, and measuring progress. New and innovative strategies developed by the states may provide inspiration and guidance to officials in other states. This event is made possible through the generous support of The Commonwealth Fund.
