New CHIPRA Opportunity: Express Lane Eligibility

The Children’s Health Insurance Program Reauthorization Act of 2009 (CHIPRA) provides states with new tools to reach, enroll and retain eligible but unenrolled children in Medicaid and CHIP. One such tool is the option to develop and implement Express Lane Eligibility (ELE), which allows states to enroll and renew children in Medicaid and CHIP based on eligibility findings of other government agencies.

While a number of states had begun to pursue automated strategies to increase enrollment prior to CHIPRA, the ELE provision is expected to provide states with opportunities to further reduce existing barriers and simplify enrollment and renewals. With the support of the David and Lucile Packard Foundation, the National Academy for State Health Policy (NASHP) presented this web seminar to explore the ELE option.

The webinar includes an overview of the ELE option in CHIPRA, including a look at early experiments with ELE that fed into how the law was drafted. We heard from a national expert who has studied the experiences of other public programs’ use of automated strategies and who explored how states can use automation, and the options in ELE, to identify, enroll and retain children in health coverage. Finally, we learned about two states’ current efforts to implement ELE, including the initial steps they took prior to CHIPRA and their expectations for results using the new tools in CHIPRA.

Gearing Up for the Citizenship Documentation Requirement in CHIP: Successful Strategies from Medicaid

The Children’s Health Insurance Program Reauthorization Act of 2009 (CHIPRA) extends the citizenship documentation requirement in Medicaid to CHIP. Beginning on January 1, 2010, CHIP programs were required to verify the citizenship and identity of new applicants and existing enrollees. States need to decide whether to verify citizenship and identify using the procedures currently employed in Medicaid or to verify citizenship and identity using a new automated data matching process developed by the Social Security Administration (SSA) pursuant to section 211 of CHIPRA. Even states that decide to implement the new SSA option may need to use the current verification process for children not found in the SSA database.

This web seminar focused on the successful strategies state Medicaid programs have developed to verify citizenship and identity since Congress first mandated that they do so. Speakers provided an overview of the previous documentation requirement, including changes to it made by CHIPRA; discussed successful strategies that different Medicaid programs across the country have used to verify citizenship and identity of children; and explored in-depth how two states have worked to minimize the burden imposed on both states and families.

Past, Present, and Future: A Report on State CHIP Programs

Charting CHIP IV: A Report on State Children’s Health Insurance Programs Prior to Major Federal Policy Changes in 2009 and 2010, NASHP’s fourth comprehensive report of states’ Children’s Health Insurance Programs (CHIP), is a detailed look at CHIP programs as of 2008. Given that this report examines CHIP programs prior to passage of the CHIP Reauthorization Act (CHIPRA) and the Patient Protection and Affordable Care Act (ACA), it provides a baseline from which to measure changes due to these laws. This webinar highlighted findings from the Charting CHIP IV report. It also highlighted just a few of states’ many accomplishments in implementing provisions of CHIPRA, including Alabama's use of the new, less burdensome citizenship documentation system; how Nebraska implemented new oral health coverage provisions; and Colorado's path to receiving a CHIPRA performance bonus. Finally, the webinar focused on the future of children’s coverage and how states can build on the successes of the CHIP and Medicaid programs to improve coverage and quality for children under the Affordable Care Act.

The Affordable Care Act and Children with Special Health Care Needs: An Analysis and Steps for State Policymakers

Approximately one in every seven children under 18 years of age, or 14 percent of children in the United States, has a special health care need. Children with special health care needs (CSHCN) have or are at increased risk for a chronic physical, developmental, behavioral, or emotional condition and also require health and related services of a type or amount greater than that required by children generally. The report, The Affordable Care Act and Children with Special Health Care Needs: An Analysis and Steps for State Policymakers, developed by the National Academy for State Health Policy (NASHP) for the Catalyst Center at Boston University reviews and analyzes key provisions of the Affordable Care Act (ACA) relevant for CSHCN to help inform state policymakers' decisions. This webinar presented highlights from the analysis, including steps that state policymakers can take to address the needs of CSHCN as states work on health care reform. A panel of state and federal officials shared their perspectives on the report.

What a Difference a Dollar Makes: Affordability Lessons Learned from Children's Coverage
States are responsible for on-the-ground implementation of the Affordable Care Act (ACA), including expanding coverage options through Exchanges, Medicaid and other health insurance programs. This webinar considers different ways policymakers define affordability and draws on lessons from the Children's Health Insurance Program (CHIP), which can serve as a model for states as they implement affordability provisions in ACA. Children’s Buy-In Programs

A buy-in program for children allows families with income greater than a state’s Medicaid and state children’s health insurance Program income eligibility levels to purchase coverage through a public plan for their children. A number of states have implemented child buy-in programs. National children coverage experts provide an explanation of children’s buy-in programs and consider the pros and cons of this coverage strategy.

This state-only webinar presented by NASHP with support from the Commonwealth Fund and the David and Lucile Packard Foundation for EPSDT and CHIP program administrators provided participants with an opportunity to learn more about the concept and legal definitions of pediatric medical necessity and to talk with their peers about implementation of pediatric necessity within Medicaid/EPSDT and CHIP programs. It also addressed working with stakeholders to develop the pediatric medical necessity definition.
