On June 2nd and 3rd, 2010, the National Academy for State Health Policy (NASHP) convened a forum to examine evolving model approaches as well as surface new strategies that can be promoted with federal and state policymakers, foundations and the health center community to help progression toward reformed health systems. This forum for participants and key partners in NASHP’s National Cooperative Agreement with the Health Resources and Service Administration Bureau of Primary Health Care included group learning sessions as well as facilitated breakout sessions to discuss issues, brainstorm approaches, and identify follow-up strategies to work toward a transformed delivery system. Below is a link to a paper based on the vision outlined at the forum for a re-formed delivery system and the strategies identified by participants as to how this vision can be achieved as well as links to the presentations that took place at the forum.
