Several decades ago when states began developing systems of community based long term services and supports, they were primarily focused on enabling people to continue living in their own homes and avoid entering an institution. Recently, state and federal officials have also focused on initiatives to help residents of nursing homes and other long term care institutions transition to the community, accompanied by array of supports they need to live independently.
With funding from the Centers for Medicare and Medicaid Services, NASHP is helping states implement the Money Follows the Person Demonstration. It has two components: 1) a transition program to enable long-term institutional residents to move back to the community and (2) a rebalancing initiative in which states use enhanced Medicaid matching funds to reduce the reliance of their long term care systems on institutional care. NASHP is developing written resources and providing targeted technical assistance to individual states.
Our work on the Money Follows the Person Demonstration builds upon resources we developed to help states successfully implement the federal forerunner program—Nursing Home Transitions.
